Complete Online Renewal Form
Log into your provider portal account and complete the renewal form online. Please review all information and fill out completely so the database can be updated and current.

Verify CE Requirements
Ensure you have completed all continuing education requirements for the renewal period.

Submit Payment & Form
Sign the form and mail it along with your $200.00 renewal fee to the Board office.

Important Reminders
Renewal Cycle
- • Licenses expire December 31st every year and must be renewed annually
- • Renewal opens October 1st each year in the Provider Portal
- • Renewal must be received by December 20th
- • Current CE cycle: 2026-2027 — 40 hours, verified on the renewal filed in the final year of the cycle
- • Late renewals may incur penalties
